Access to Medical Records
The practice is registered and complies with the Data Protection Act 1998. Any request for access to notes by a patient, patient’s representative or outside body will be dealt with in accordance with the Act. Please contact the Practice Manager for further information. Your health records contain a type of data called confidential patient information. This data can be used to help with research and planning. You can choose to stop your confidential patient information being used for research and planning. You can also make a choice for someone else like your children under the age of 13. Your choice will only apply to the health and care system in England. This does not apply to health or care services accessed in Scotland, Wales or Northern Ireland. Information about you, the services you receive and your family background may be recorded either on paper or computer files as part of providing you with health and social care services. We keep records of: Anyone who receives patient identifiable information from us is also under a legal duty to keep it confidential. We take the sensitivity of personal information very seriously in the Practice. Staff and contractors are required to respect their duty of confidentiality, codes of conduct and through their contracts of employment. Sharing of information is strictly governed by information protocols and the NHS Code of Practice for confidentiality. These are produced in accordance with the legislation and good practice and are designed to protect your rights. We are also registered under the Data Protection Act and adhere to the General Data Protection Regulations (GDPR). We abide by all statutory requirements regarding confidentiality, whilst following the appropriate guidelines for information security. All our staff are required to work within these guidelines. Information Security
Complaints and compliments
We always try to give the best possible service to our patients but accept that there may be times when you feel this has not happened. We hope you will use it to allow us to look into and, if necessary, put right any problems you have identified or mistakes we have made. If you us this procedure, it will not affect your statutory right to take the matter to the Health Service Ombudsman. We ask that you put your complaint in writing addressed to our Complaints Manager We know it is important to deal with complaints swiftly. We will acknowledge receipt within 3 working days and, depending on the severity of the complaint, you will normally be offered a response within 10 working days. Occasionally, if there are a lot of enquiries to be made this may take a little longer, but we will always keep you informed. We will make every effort to fully address your concerns and we hope that by the end of the process you will be satisfied that the matter has been dealt with thoroughly. GP Net Earnings
Named GP
All patients at Glebe Park Surgery have a named, accountable doctor who is responsible for coordinating their care.
Your named doctor will be allocated to you by the practice. You can still talk to or make appointments to see any of our doctors or nurses, not just your named GP.
If you have a preference and would like to request a particular doctor at the practice to be your named GP please talk to one of our receptionists.
Patient Confidentiality
We respect your right to privacy and keep all your health information confidential and secure. It is important that the NHS keeps accurate and up-to-date records about your health and treatment so that those treating you can give you the best possible care.
This information may be used for management and audit purposes. However, it is usually only available to, and used by, those involved in your care. You have the right to know what information we hold about you. If you would like to see your records please contact the Practice Manager.
Freedom of Information
Information about the General Practitioners and the practice required for disclosure under this act can be made available to the public. All requests for such information should be made to the practice manager.
Summary Care Record
There is a new Central NHS Computer System called the Summary Care Record (SCR). It is an electronic record which contains information about the medicines you take, allergies you suffer from and any bad reactions to medicines you have had.
Why do I need a Summary Care Record?
Storing information in one place makes it easier for healthcare staff to treat you in an emergency, or when your GP practice is closed.
This information could make a difference to how a doctor decides to care for you, for example which medicines they choose to prescribe for you.
Who can see it?
Only healthcare staff involved in your care can see your Summary Care Record.
How do I know if I have one?
Over half of the population of England now have a Summary Care Record. You can find out whether Summary Care Records have come to your area by asking your GP
Do I have to have one?
No, it is not compulsory. If you choose to opt out of the scheme, then you will need to complete a form and bring it along to the surgery. You can use the form at the foot of this page.

Violent or Abusive Behaviour
We take seriously any threatening, abusive or violent behaviour against any of our staff or patients. If a patient is violent or abusive, they will be warned to stop their behaviour. If they persist, we may exercise our right to take action to have them removed, immediately if necessary, from our list of patients.
